Student-Athlete Required Participation Checklist & Forms
In order for a student-athlete to be cleared to participate in any and all athletically-related activities (practice, competition, tryouts, strength & conditioning, etc.), they must complete the following:
Teamworks
- Teamworks is a platform/app that helps our department function. It is essential that student-athletes learn to use it. To begin using Teamworks, you should first:
- Download the TW 2.0 app (not the “Teamworks” app) from the app store, select “Sign in with SSO,” type “arcadia” in the subdomain, and use your Arcadia SSO credentials (this is your Arcadia login).
- The app is the most convenient way to use Teamworks as it allows you to receive live notifications on your mobile device. In addition to the app however, you can also access Teamworks through the online platform:
- Go to arcadia.teamworksapp.com, select “Sign in with SSO,” and use your Arcadia SSO credentials. In the event that you land in Arcadia’s Portalguard after logging in, simply click on the Teamworks tile as things may look different on your first attempt.
Athletic Training
- All clearance needs are managed in the Teamworks app. In the app, after selecting AMS, you will see a “Forms” tab and a “Dashboards” tab. With the “Forms” tab selected please use the “Pre-Participation Questionnaire Hub” to complete and/or upload all required forms. Once you click “Pre-Participation Questionnaire Hub,” you will see a list of forms appear.
- To complete forms, select “view entries” under each listed form. At the bottom of the newly loaded page you will see “Add New Entry” to access and complete each form. You will have the ability to Save forms and return to them, but should click Submit when the form is complete.
- If you are having trouble accessing forms on your mobile device, we strongly encourage you to access your Teamworks account on a computer by clicking here.
- Once logged in, click on Teamworks Apps (bottom left corner) and switch to AMS. Withing AMS you will see an “Enter New Forms Here” tile - select “Enter Data.” From here you will see all required medical forms listed. Complete each form by selecting the corresponding grey bar labeled “New ____ Form.” You will have the ability to Save forms and return to them, but should click Submit when the form is complete.

Athletic Communications
- Complete ONLINE questionnaire for new or returning student-athletes on Teamworks.
Compliance
- Complete required NCAA and Arcadia compliance forms online through the Teamworks app prior to arrival on campus
- Attend a mandatory compliance session in person (see your head coach for dates and times)
- Review the following supplemental materials on your own:
- Transfer students, international students, and students delaying college enrollment after high school graduation must be identified and will need to complete additional compliance clearances
- NCAA ID’s Required - Please keep your 10-digit ID number accessible as we will collect it at a later time
- In accordance with NCAA regulations, we are required to collect NCAA ID numbers for all incoming student-athletes, including transfer students who will be joining Arcadia for the first time this year and in future terms (returning student-athletes do not need to create an account). All new, incoming students are required to have an ID

Academics
- Meet all academic standards for full-time enrollment, satisfactory academic progress, and good academic standing
Current Arcadia University student-athletes interested in transferring to another Division III institution can grant their own Permission to Contact using the NCAA Division III Permission to Contact: Self-Release. Student-athletes may also utilize the transfer portal to explore options at Division I, Division II and Division III, however, you must meet with the Compliance Officer in order to be entered into the transfer portal.
